Getting to Requisition Form

Step 1: Sign into Banner Self-Service

Step 2: Select “Finance

Step 3: Select “My Requisitions

Step 4: Select “Create Requisition

Tab 1: Requestor Information

Step 1: Check to make sure the Requestor, Transaction Date, and Requestor Email Address have automatically populated with your information and today’s date.

Step 2: Add a Delivery Date

Step 3: Public Comment – only enter in this field if you have specific instructions for Procurement (ex. email the PO to jaysmith@arco.com)

Step 4: Private Comment – only enter in this field notes for your records only. Private comments will not appear on the purchase order and cannot be viewed by anyone else.

Step 5: Chart of Accounts – always use “L

Step 6: An Organization will populate for your department. If you need to use a different Organizations, press the down arrow to see which Organizations you have access to.

Step 7: Ship To Location – Your department Ship To will populate. If you need to change the Ship To location, press the down arrow to view other Ship To Locations that you have access to.

Step 8: Attention To – Must enter a person’s name in this field

Step 9: Tax Group – (DO NOT ENTER ANY INFO HERE)

Step 10: Click the “Next” button at the bottom of the page to move on to Tab 2

 

Tab 2: Vendor Information

Step 1: Vendor – Begin entering a vendor’s name and it will show all vendors beginning with the first few letters of the name. Choose the correct vendor’s name and it will populate. (Make sure to select correct address if multiple show up. Also, use the little “x” next to vendor if you make the wrong selection and need to reset.)

Step 2: Vendor Email – If the vendor has an email contact on file, that will populate below the address after a vendor is selected. You can also manually enter an email contact here.

Step 3: Discount – Always choose “02 Net 30 Only

Step 4: Currency – Always use “USD” only

Step 5: Click the “Next” button at the bottom of the page to move on to Tab 3

 

Tab 3: Add Item & Accounting

Step 1: Add item – Enter type of item (ex. Fire alarms) and the category of item will appear, choose the best choice

Step 2: Unit of Measure – Choose most appropriate (ex. each = EA; group = GRP)

Step 3: Tax Group – (LEAVE BLANK)

Step 4: Quantity – Enter how many items are being purchased

Step 5: Unit Price – Enter Price per Unit

Step 6: Discount, Add Amount, & Tax Amount – (DO NOT ENTER ANYTHING IN THESE FIELDS)

Step 7: Public comment – Enter a good description of the item being ordered including the quote number and the quote date

Step 8: Private Comment – only enter in this field notes for your records only. Private comments will not appear on the purchase order and cannot be viewed by anyone else.

Step 9: Click on “Add Accounting” at the bottom

Step 10: Your department’s Fund and Organization will auto populate (if you need to select a different Fund/Organization use the drop-down arrow to see which ones you have access to and select the most appropriate)

Step 11: Select the Account Number (Ex. 710357)

Step 12: The Program code with auto populate as well

Step 13: If an Activity code & Location code also need to be added, select the most appropriate

Step 14: Distribution Amount and Distribution Percent – will auto populate with the total of your Requisition and 100% distribution to the Account number selected in Step 11.

Step 15: If the Distribution Amount is to be split between multiple Account Numbers, change the Distribution Amount to how much goes to this Account number selected in Step 11 (this will give you the option to “Split Accounting”). Click “Split Accounting” to select the next Account number and add the next Distribution Amount.

Step 16: Click the “Save” button at the bottom of the page

Step 17: If you need to add another Item, use the Add Item drop-down and repeat Steps 1-16

Step 18: You will see an R# assigned to your Requisition. Select “Submit Requisition” if all looks good. (There is also a “Save as draft” option if you are not ready to submit yet).
